Who We Contract With

Our agency is dedicated to providing compassionate and reliable support for individuals in need. We are an approved Medicaid provider for Housing Support and other Dept of Disability (DDD) services and are fully authorized by all five MCOs to deliver care.

  • Living in situations where you lack a fixed, regular, and adequate night-time residence.
  • Fleeing or attempting to flee domestic violence, with no other residence available.
  • A youth under 25 who qualifies as homeless under federal statutes.

Are You At Risk of Homelessness?

You may be at risk if you are:

  • Couch surfing, facing eviction, or living in overcrowded conditions.
  • Living in a physically unsafe home.
  • At risk of institutionalization or transitioning out of an institution.
  • Recently released from a correctional facility.
